power of pethidine!!!

tetiba teringat al kisah aku oncall satu hari tu...

tiap kali oncall mesti berdoa dan berharap akan dapat one peaceful oncall....peacefull oncall maksudnye admission tk banyak or dapat tido malam tu...itis a bless if u could sleep during oncall...hehe

siang tu dalam kul 6 petang ( 6 petang semua HO yg tak oncall dah cabut balik umah) ade patient tu masuk.....

41/lady
c/o : RIF pain x 2/7
- sudden onset
-pricking in nature
-continuous pain
- no radiation,,no relieving,,,aggravate with movement....

+vomit x 4 times
+diarrhoea

+ feeling feverish

on examination
-soft, tender at RIF on deep palpation
-fever 38degree celcious

then seen by MO,,,with plan :

1. keep NBM ( nil by mouth )
2. no need analgesic yet
3. IVD 3 pint NS
4. to inform if patient temperature high or pain increase

erm...looking of that i thought....OMG!!!! it going to be appendix day!!!!!after that my MO left the ward......so tinggalla saya keseorangan bersama my nurses

after that i 'm waiting for the next patient......( dekat hati ni dah berkata kata..." janganla ade patient lagi...." )

IT'S TRUE!!! takde new admission that nite...TAPIKAN........

at 10 pm : " doktor patient sakit perut kuat...( patient appendix juge)

at 11.pm : " ooo...explain to patient abuot her condition....no need analgesik yet..

at 12 midnight : doktor,,,sakit sgt doktor.....bile mau operation????? erm...pagi esok ye auntie,,,

aku pun palpate abdomen dia....guarding and tender generalised all abdomen..+ rebound tenderness,,,+ peritonitis....

gile la...dah sudah..mati la aku...huhu.....dah jadi perforated apendiliciltis,,huhu...

terus tepon MO oncall " doktor patient bla,,,bla,,bla,,bla,,," so MO dengan bunyi mengantuk dia order " give iv tramal 50mg stat dose..."

aku pun bagi la as order by my MO...

at 1 am : patient mengerang kesakitan walaupun selepas iv tramal.......

mata ni dah terbuka tertutup dah tp di gagahkan juga sebab patient tu sakit,,,,mengerang kesakitan,,,tak senang duduk,,, tak leh tido.......sepanjang malam menguruskan dia.....patient lain aku tgk dh tido dgn selesanye... ( JELESNYE AKU!!!! )

at 2 am : calling my MO again..." sorry to disturb u again doktor....patient in severe pain,,,,abdomen intense gurading,,,tender the whole abdomen...erm...dengan coolnye doktor dah cakap....

" GIVE PETHIDINE TO PATIENT "

aku pulak,,,," kak nurse,,,pethidine dose berapa ek??huhu....aku pun bagi la patient petidine 50 mg stat....

aku dgn suke hatinye abgi patient phetidine.... guest what?????

patient was well...no more in pain...

PATIENT TELAH TIDUR after IV PETHIDINE 50 MG TDS....

patient tido sound ly.....huhu..maksdunye aku pun malam tu dapat peaceful oncall n tido soundly smpai pagi....huhu..

dalam hati ku berkata " pethidine mmg power!!! huhu....."

keesokan harinye patient tu terus kena appendicectomy...mmg dah perforated pun....so terpaksa la patient stay 7 hari lgi kt ward nk habiskan IV antibiotic selama 7 hari....huhu...arini..aunty akan balik kerumah...aunty kate,,,"boringla doktor duduk ward lama lama...." haha......
